Please refer to the documents below: Work Permit Application, and Physical Fitness Certification.  

• Students may either pick up these documents from their school offices during the school year, or can fill them out online & print them.  
• Students and parents/guardians must complete Part 1 of the Permit Application Form and have their physician complete the Physical Fitness Certification Form & turn the completed forms back to their school office.
•  If the certifying officer needs any additional documentation, the student will be contacted.
